14 violations, Domino’s in Rockledge; “Facility instructed that it MUST close while grouting is taking place, and MUST remain closed until grout has dried…”

The Pennsylvania Department of Agriculture’s Bureau of Food Safety and Laboratory Services conducted an unannounced retail food inspection on 1/20/21. As a result of that inspection, and due to the severity of the specific violations Domino’s Pizza was cited as being, “Out of Compliance” with Pennsylvania’s food safety regulations. During this inspection a total of 14 violations were found.

The inspector made the following comments in support of the violations found during the inspection:

  • (5 times repeat violation)Facility does not have current food operational license on site at time of inspection. The Facility must have current original food license posted in public view. Locate or request a duplicate copy from MCOPH. Failure to correct and maintain multiple repeat violations may result in further legal action. Citation possible.

  • (4 times repeat violation)Sign stating inspection reports are available upon request not posted in public view. Sign emailed during inspection. Failure to correct and maintain multiple repeat violations may result in further legal action.

  • Facility CFSM certificate not posted publicly at time of inspection.

  • Utensils, hoses, and other items observed obstructing handwashing sink basins in kitchen and ware washing room. Handwashing sinks are to be used for handwashing only.

  • Employee entrance door propped open at time of inspection.

  • Three compartment sink is being supported by raw plywood as its’ legs have been removed due to facility retiling ware washing room.

  • The Facility is in the process of re-tiling ware washing room. Upon entering the facility, employees stated that they could not use the three compartment sink as the repairs to the tile flooring were not complete. The Facility supervisor arrived during inspection and stated that employees are able to access the sink at this time, and that a contractor would be back this afternoon to complete grout work and complete all necessary repairs in the ware washing room. Facility instructed that it MUST close while grouting is taking place, and MUST remain closed until grout has dried and the area is accessible. Failure to close during this time may lead to immediate legal action including citation and/or closure. A 24-hour follow-up will be conducted to ensure that repairs have been completed.

  • Mop sink removed and not in place due to facility retiling ware washing room floor. The Facility is hereby given 24 hours to fully re-install mop sink. A 24-hour follow-up inspection will be conducted to ensure that the sink is installed. Failure to comply within established time frame may lead to legal action, citation and/or closure.

  • Handwashing sink in ware washing room is currently not plumbed to drain. The Facility must re-connect the basin of sink to plumbed drain. A 24-hour follow-up inspection will be conducted to ensure that repair is made. Failure to comply within established time frame may lead to legal action, citation and/or closure.

  • Self-closing device on employee restroom door no longer operational.

  • Dumpster located to rear of facility observed lacking lid.

  • Cardboard debris observed throughout exterior dumpster area enclosure.

  • Cove base removed in ware washing room due to facility retiling floor. The Facility must re-install cove base in areas in which it was removed or is missing.

  • Mop not hung or inverted to dry following use.
