The Montgomery County Health Department conducted an unannounced retail food inspection on 1/25/21. As a result of that inspection, and due to the severity of the specific violations the Bullseye Burger House in Glenside was cited as being, “Out of Compliance” with Pennsylvania’s food safety regulations. During this inspection a total of 38 violations were found.

The inspector stated in the report, “Facility has been scheduled for a re-inspection on or after 2/8/21. Should facility not be in compliance at the time of re-inspection they will be required to attend an administrative conference and will receive an additional re-inspection. Facility will be required to remit a re-inspection fee $330.00 if given an administrative conference.”

The inspector made the following comments in support of the violations found during the inspection:

  • Facility lacking sign stating inspection reports available upon request posted in public view. Sign emailed to facility during inspection.
  • (2 times repeat violation)Facility is lacking a written vomit/fecal incident procedure at time of inspection. Sample document emailed during inspection. Failure to correct and maintain multiple repeat violations may lead to legal actions.
  • No paper towels provided at front area hand wash sink.
  • (3 times repeat violation) No paper towels provided at back prep area hand sink. Failure to correct and maintain multiple repeat violations may lead to legal actions.
  • No soap provided at back prep area hand wash sink.
  • No hand wash sign posted at back prep area hand wash sink.
  • Front area hand sink lacking hot water supply. A 24-hour follow-up inspection will be conducted pending repairs to verify compliance.
  • Back prep area hand sink found completely obstructed by food tray stacked with cans.
  • Food items in refrigeration and freezer units throughout facility found uncovered.
  • Raw shell eggs stored above produce in 2 door tall refrigerator in back.
  • Chlorine sanitizer bucket tested above 50-100ppm concentration. Facility diluted solution to 50ppm during inspection.
  • At the time of inspection the facility did not have 3 compartment sink set up properly. Basin filled with soap was labeled sanitizer and other end of sink basin had a large white bucket inside of it being used as sanitizer. Facility instructed on proper wash, rinse, sanitize, and air dry procedures.
  • At time of inspection facility lacking any chlorine bleach sanitizer solution on site. Facility obtained sanitizer during inspection and was instructed that sanitizer must be made available at all times of operation.
  • Foods on grill top found to be below 135F. The Facility was instructed that if they wish to hold food outside of temperature control a time temperature log must be used. Facility instructed on log use during inspection.
  • (5 times repeat violation) Prepared and opened TCS ready to eat foods held in facility under refrigeration longer than 24 hours missing required discard date marking not to exceed 7 days from the prep/open date. Facility emailed date marking regulations. Failure to correct and maintain multiple repeat violations may lead to legal actions.
  • Facility found to be storing raw bacon at room temperature during service. Facility was instructed that if they wish to hold food outside of temperature control a time temperature log must be used. Facility instructed on log use during inspection.
  • The Facility has placed a consumer advisory statement on menus but does not specify on the menu which items are cooked to order by an asterisk or other means of designation.
  • Employee prescription medication bottle stored on salad prep refrigerator.
  • Several blocks of open rodent bait/poison found on ground across from mop sink. All rodent bait must be properly placed inside of a rodent bait station at all times.
  • (2 times repeat violation) Facility lacking a probe thermometer to take internal food temperatures. Failure to correct and maintain multiple repeat violations may lead to legal actions.
  • (4 times repeat violation) Rodent-like droppings observed in area across from facility mop sink, around mop sink, and on floor under equipment and shelving in kitchen areas. Facility must clean and sanitize area and have licensed pest control service. Send pest control report to fax or email provided within 10 business days. Failure to correct and maintain multiple repeat violations may lead to legal actions.
  • Cooked bacon stored in direct contact with absorbent cloths.
  • Facility storing raw bacon on clean side of 3 compartment sink.
  • The Facility has installed a non-food grade hose and sprayer onto their food prep sink. A 24-hour follow-up inspection will be conducted to verify proper installation of a food grade hose and/or sink faucet.
  • (5 times repeat violation) Food handler lacking hair restraint. Failure to correct and maintain multiple repeat violations may lead to legal actions.
  • (2 times repeat violation) Knives stored between prep table and cook’s line prep refrigerator. Failure to correct and maintain multiple repeat violations may lead to legal actions.
  • Boxes of single use food containers stored on floor in storage area.
  • (2 times repeat violation) Many surfaces and shelving observed lined with absorbent cloths and absorbent cardboard along cook’s line. Failure to correct and maintain multiple repeat violations may lead to legal actions.
  • Cardboard material found taped to side of milkshake machine.
  • Facility found to be re-using empty water bottles to store sauces in.
  • Facility lacking chlorine test strips.
  • 3 compartment sink left sanitize basin observed with a drip leak.
  • Facility mop sink basin obstructed by large box.
  • Inadequate air gap drain observed under facility food prep sink. Pipe hanging deep into drain cup.
  • The Facility stated they do not use their mop sink. The Facility must use this sink for approved discharge of mop/waste water and cannot pour mop water on the ground out the back door of the facility.
  • Cook’s line grease hood observed with grease/soot accumulation.
  • Facility storing mops inside of soiled mop water in mop buckets.
