The Pennsylvania Department of Agriculture’s Bureau of Food Safety and Laboratory Services conducted an unannounced retail food inspection on 7/29/2020. As a result of that inspection, and due to the severity of the specific violations the Five Iron Grille was cited as being, “Out of Compliance” with Pennsylvania’s food safety regulations. During this inspection a total of 22 violations were found.

The inspector made the following comments in support of the violations found during the inspection:

  • The Person in Charge does not have adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection.
  • Employee beverage stored above open food packages in the upright refrigerator in the kitchen area.
  • Observed steak, breaded cheese wedges, and seafood in the single-door upright freezer, located in the dry storage area, in an adulterated state due to spoilage and mold.
  • Various foods stored in the single-door upright refrigerator (sausage, sliced cheese) and freezer (french fries, breaded chicken), located in the kitchen area, stored open with no covering.
  • Observed a clear plastic condiment bottle in the kitchen area not labeled to identify the contents
  • Observed bagged potato chips, bottled vinegar, and reusable condiment bottles stored on/next to/below a water drainage pipe system. The system included an air break, subjecting those items to the possibility of splash from the drainage water.
  • Sausage, sliced deli meat and cheese – all ready to eat, time/temperature control for safety foods – located in the single-door upright refrigerator and the refrigerated sandwich preparation table, are held more than 24 hours, but are not being marked with the date they were opened.
  • Facility utilizes both chlorine and quaternary sanitizers. The chlorine sanitizer testing strips that were available at the time of inspection had a manufacturer’s expiration date printed on the label of July 2018.
  • Observed deeply scored cutting boards not resurfaced or discarded as required.
  • Employee personal items (backpack) stored on a food preparation table and next to kitchen utensils. **CORRECTED**
  • Observed mold-like residue accumulation on the bottom shelf of the double-door refrigerator located in the back storage room.
  • Observed old food residue on the walls of the ware washing area.
  • The food facility does not have the original certificate for the certified food employee posted in public view.
  • The handwash sink located in the ware washing and food preparation area is out of service. The available handwash sink, located at the bar in the front of the facility, is not convenient and easily accessible for the ware washing and food preparation areas.
  • Observed a leak at the cold-water handle of the 3-bay sink in the ware washing area.
  • No sign or poster posted at the handwash sink in the bar area to remind food employees to wash their hands.
  • Rodent droppings on the floor and along the walls throughout the back storage area, including the dry storage room – too many to count.
  • Observed approximately 3 small winged insects exit the holster for the soda gun nozzle nearest the handwash sink of the bar area.
  • Food facility is not removing dead or trapped pests from control devices frequently, as evidenced by observed old / decaying pests in the control devices located in the back storage area.
  • Air ducts in the kitchen area need to be cleaned as leaves and debris were observed caught in the registers.
  • Mops are not being hung to air dry.
  • Observed canned spray cleaner stored on a shelf above knives and a food preparation table in the kitchen area. **CORRECTED**
  • Observed cleaning chemicals and sanitizers stored directly above the 3-bay sink and a rack of kitchen utensils. **CORRECTED**
