Long John Silver’s/A&W Restaurant fails inspection in Rochester;  Black bio-film on ice chute food contact surface, ice thrown out, 6 violations

The Pennsylvania Department of Agriculture’s Bureau of Food Safety and Laboratory Services conducted an unannounced retail food inspection on January 8, 2020. As a result of that inspection, and due to the severity of the specific violations the Long John Silvers, A&W in Rochester was cited as being, “Out of Compliance” with Pennsylvania’s food safety regulations. During this inspection a total of 6 violations were found.

The inspector made the following comments in support of the violations found during the inspection:

  • Three knives, which are food contact surfaces, were observed to have food residue and was not clean to sight and touch. These were stored in knife holder. Corrected on site.
  • The interior chute of the ice machine, a food contact surface, was observed to have a black bio-film and was not clean to sight and touch. Ice was discarded and unit corrected on site.
  • Salmon (47°F), shrimp (46°F), shredded lettuce (45°F) and sliced tomatoes (46°F) in the Delfield unit, rather than 41°F or below as required. Shrimp, tomatoes, and shredded lettuce were placed into unit at 8:00AM. The shrimp, tomatoes, and shredded lettuce were relocated to alternate cooler. Salmon was discarded due to undetermined time in this unit. Ambient air of prep unit is 49.8°F. Discontinue use of unit until it is repaired or replaced.\ Thermometer field-calibrated as witnessed by the manager.
  • Food contact surface of spatula is not smooth, easily cleanable and is cracking and missing pieces. Corrected on site.
  • Trash receptacle handling unit outside does not have the lid closed. Debris, fryer crispy’s, present on surrounding ground.
  • Ceiling exhausts, notably outside of the hood area, but found in all locations, of the food facility are extremely dirty, dusty, and in need of cleaning.
  • Loose rubber door gaskets observed on the Delfield prep unit in prep area.
