The Montgomery County Department of Health and Human Services Office of Public Health (MCOPH) conducted an unannounced retail food inspection on DATE. As a result of that inspection, and due to the severity of the specific violations Salad Works in the Willow Grove Mall was cited as being, “Out of Compliance” with Pennsylvania’s food safety regulations. During this inspection a total of XX violations were found and include one or more violations determined to be food borne illness risk factors.

The inspector made the following comments in support of the violations found during the inspection:

  • (5 times repeat violation)Facility lacking a CFSM certificate with MCOPH. Facility is granted 5 business days to apply or provide proof of course enrollment to fax or email provided. Application emailed during inspection. Failure to comply may result in issuance of a citation due to repeat non-compliance.

  • (2 times repeat violation)Employee beverages stored on prep table and on top of oven. Failure to correct and maintain multiple repeat violations may result in further legal actions.

  • Employee partially consumed ice cream cup found stored on boxes of foods for customers in 2 door tall freezer.

  • No soap provided at back ware washing area hand sink.

  • 3 compartment sink with no detectable quaternary sanitizer concentration. Facility remixed solution to 200ppm during inspection.

  • (2 times repeat violation)Exposed rodent poison bate blocks observed scattered on floor throughout facility. Remove bate blocks that are loose. Poison bait blocks must be contained in bait stations. A follow-up inspection will be conducted on 2/26/20 to verify removal of all exposed rodent bait. Failure to correct and maintain multiple repeat violations may result in further legal actions.

  • (7 times repeat violation)Rodent-like droppings observed on floor throughout front and back areas of facility in corners and under/around equipment. Droppings also observed on some equipment and around hot water heater. Facility must clean and sanitize all areas and have a licensed pest control service within 5 business days. Pest control report must state treatment of target pest. Send pest control report to fax or email provided. A follow-up inspection will be conducted on 2/26/20 to verify clean up of evidence of pest activity. Failure to correct and maintain multiple repeat violations may result in further legal actions.

  • Food items stored in cabinet under hand washing sink drain pipe at front counter area.

  • Shelving in walk-in refrigerator does not elevate foods a minimum of 6 inches from floor.

  • Wet wiping cloths are not being stored in sanitizer solution between uses.

  • Single use food/beverage items stored in cabinet under hand washing sink drain pipe at front counter area.

  • Door gasket of 1 door tall refrigerator by walk-in unit observed damaged and held on door with tape materials.

  • Raw wood shelf observed under oven in back prep area. Provide smooth, non-porous, easily cleanable surface.

  • Facility lacking quaternary test strips.
