US Gas in Lebanon hit with 12 violations; bottom of the sink covered with black dirt and residue and wet sponge that was black

The City of Lebanon Board of Health conducted an unannounced retail food inspection on December 17, 2019. As a result of that inspection, and due to the severity of the specific violations the 8TH ST. US GAS was cited as being, “Out of Compliance” with Pennsylvania’s food safety regulations. During this inspection a total of 12 violations were found.

The inspector made the following comments in support of the violations found during the inspection:

  • Observed bread and rolls in cartons stored directly on the floor in the freezer area, rather than 6 inches off of the floor as required.
  • The ice scoop, a food contact utensil, is sitting on top of the ice machine in an area that is rusted and stained.
  • The third compartment of the 3-compartment sink observed in filthy condition. The bottom of the sink was covered with black dirt and residue and contained a wet sponge that was black.
  • Facility is not using sanitizer on equipment and utensils after washing and rinsing them, and prior to use.
  • Commercially processed ready to eat food, located in the walk-in cooler, and held more than 48 hours, is not being marked with the date it was opened.
  • Some shelves in the walk-in cooler are badly deteriorated and no longer able to be cleaned properly.
  • Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil; specifically plastic shelves for half gallons in the walk-in cooler.
  • The inside of the microwave, a food contact surface, was observed to have food residue and was not clean to sight and touch.
  • A glass light cover over the light bulb in the walk-in freezer is cracked. The metal guard that should be over the cover is missing.
  • Four quarts of milk were dated 12/14, three 14-oz. containers of milk were dated 12/09, and one 14-oz. container of milk was dated 11/29. Discarded.
  • BBQ sandwich, pizza and burritos were held at 75°F to 78°F, in the food service area, rather than 135°F or above as required.
  • Mop is not being hung to air dry; it is sitting in the mop bucket.
