20 violations for Bullseye Burger House in Glenside; Rodent-like droppings observed 3 times repeat violation, Health Dept threatens legal action

The Montgomery County Department of Health and Human Services Office of Public Health (MCOPH) conducted an unannounced retail food inspection on January 15, 2020. As a result of that inspection, and due to the severity of the specific violations the Bullseye Burger House in Glenside was cited as being, “Out of Compliance” with Pennsylvania’s food safety regulations. During this inspection a total of 20 violations were found.

The inspector made the following comments in support of the violations found during the inspection:

  • Facility is lacking a written vomit/fecal incident procedure at time of inspection. Sample document emailed during inspection.

  • (2 times repeat violation)Many employee beverage containers stored on food prep surfaces, in refrigeration units, and on storage shelving throughout facility. Failure to correct and maintain multiple repeat violations may lead to legal actions.

  • No paper towels provided at front area hand wash sink.

  • (2 times repeat violation)No paper towels provided at back prep area hand sink. Failure to correct and maintain multiple repeat violations may lead to legal actions.

  • No soap provided at back prep area hand wash sink.

  • No hand wash sign posted at back prep area hand wash sink.

  • Chlorine sanitizer bucket tested above 50-100ppm concentration. Facility diluted solution to 50ppm during inspection.

  • (4 times repeat violation)Prepared and opened TCS ready to eat foods held in facility under refrigeration longer than 24 hours missing required discard date marking not to exceed 7 days from the prep/open date. Facility emailed date marking regulations. Failure to correct and maintain multiple repeat violations may lead to legal actions.

  • Facility has placed a consumer advisory statement on menus but does not specify on menu which items are cooked to order by an asterisk or other means of designation.

  • Chemicals stored above food items on back storage rack.

  • Facility lacking a probe thermometer to take internal food temperatures. Facility must provide proof of purchase to fax or email provide within 48 hours.

  • (3 times repeat violation)Rodent-like droppings observed in area across from facility mop sink. Facility must clean and sanitize area and have licensed pest control service. Send pest control report to fax or email provided within 10 business days. Failure to correct and maintain multiple repeat violations may lead to legal actions.

  • (4 times repeat violation)Food handler lacking hair/beard restraint. Failure to correct and maintain multiple repeat violations may lead to legal actions.

  • Wet wiping cloths not stored in sanitizer between uses.

  • Knives stored between prep table and cook’s line prep refrigerator.

  • Cutting board stored behind facet of 3 compartment sink.

  • Box of single use food containers store don floor in storage area.

  • Many surfaces and shelving observed lined with absorbent cloths, absorbent cardboard, paper materials, and aluminum foil throughout facility and inside of refrigeration units.

  • Facility lacking chlorine test strips.

  • Interior of microwave observed with food accumulation.
