Beaver Falls Subway in Walmart inspection finds repeat; over 20 house flies in the prep area, counter area, and customer dining area, warm tuns salad

The Pennsylvania Department of Agriculture’s Bureau of Food Safety and Laboratory Services conducted an unannounced retail food inspection on 8/12/2020. As a result of that inspection, and due to the severity of the specific violations the Subway in Walmart was cited as being, “Out of Compliance” with Pennsylvania’s food safety regulations. During this inspection a total of 8 violations were found.

The inspector made the following comments in support of the violations found during the inspection:

  • Tuna salad was prepared from food ingredients held at ambient temperature and placed on the prep table and was at 59*F after 1 hour. This is an improper way to cool foods. The tuna salad must be cooled to 41°F within 4 hours. The tuna was moved to the walk-in cooler to cool down. Facility also agreed to start cooling ambient ingredients prior to preparation.
  • Trash receptacles within the food facility that are not in immediate use are not covered properly.
  • No sign or poster posted at the handwash sink in the prep area to remind food employees to wash their hands.
  • Commercially processed meatballs were not heated to 135°F before being placed in the steam table. Meatballs had been put on the steam table 1 hr prior to the inspection and were only 76*F. Meatballs were reheated to 177*F in the microwave and placed back in the steam table. Advised manager that leftover meatballs must be reheated to 165*F for 15 seconds before being placed in the steam table.
  • Observed cooked chicken breast held at 46°F since 1 day prior to inspection, in the prep table area, rather than 41°F or below as required. Chicken breast was discarded. Other items on the prep table were held at required temperatures. Previously cited on 3/15/19 and 3/11/19. Inspector’s thermometer was field calibrated as witnessed by the manager.
  • The Person in Charge does not have adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection.
  • Soap was not available at the hand wash sink in the prep area. An alternate sink is available in the counter area.
  • Observed over 20 house flies in the prep area, counter area, and customer dining area. The fly curtain is blowing air into the facility thus inhibiting fly control measures. This violation was previously cited on 7/25/19.
