Complaint for Philly Dave and Busters; live rodent observed, 10 violations following inspection

Philadelphia’s Office of Food Protection conducted an unannounced retail food inspection on December 9, 2019 following a complaint to city officials. As a result of that inspection, and due to the severity of the specific violations the Dave and Busters was cited as being, “Out of Compliance” with Pennsylvania’s food safety regulations. During this inspection a total of 10 violations were found.

The inspector made the following comments in support of the violations found during the inspection:

  • Initial water temperatures observed at the following hand wash sinks:
    -Women’s employee restroom: 95 degrees F
    -Men’s employee restroom: 70 degrees F
    -Showroom women’s restroom: 82 degrees F
    -Beverage station in the kitchen: 75 degrees F
    -Game area bar: 70 degrees F
    -Game area women’s restroom (3 of 5): 66 degrees F
    *PIC initiated immediate corrective action; most hand wash sinks obtained a temperature of at least 100 degrees F as required prior to completion of inspection.  New Violation.
  • Live rodent observed in the ware washing area at the time of inspection.  New Violation.
  • Soap dispenser not functioning at the hand wash sink at the beverage station in the kitchen. Provide soap, paper towels, and hand wash reminder sign at the hand wash sink in the showroom bar area and party room bar area prior to operation.  New Violation.
  • 1. The Quaternary ammonia concentration in the sanitizing solution of the 3-bay ware wash sink was 150 ppm, rather than 200-400 ppm as stated on the manufacturer’s use directions in the prep area.
    2. The Quaternary ammonia concentration in the sanitizing solution of the 3-bay ware wash sink was 100 ppm, rather than 200-400 ppm as stated on the manufacturer’s use directions in the main bar area.
    *PIC took corrective action to empty 3-bay sink compartment and reset sink set ups.  New Violation.
  • Food (chicken, sauces) was held at 50 degrees F rather than 41 degrees F or below as required in one refrigeration unit.
    *PIC relocated food items to walk-in cooler to obtain proper temperature.  New Violation.
  • Inaccurate thermometer reading on some of the refrigeration units in the prep area.  New Violation.
  • Pipe connection at the ware washing area dish machine observed inefficient; excess water pouring on floor.  New Violation.
  • 1. Food debris observed along shelving in the reach-in freezer in the prep area.
    2. Food residue observed along inner surfaces of microwave oven in the prep area.
    3. Dust and debris observed along top surface of the refrigeration unit in the showroom bar area.  New Violation.
  • Grease accumulation observed along hood filters above the deep fryers.  New Violation.
  • Some excess trash and debris observed in the dumpster area; potential rodent harborage.  New Violation.
