- Ice machine, a food contact surface, was observed to have food residue and was not clean to sight and touch. Observed an accumulation of black filth in clear water tubing, on the drip tray, and on the door to the drip tray. Ice machine is to be emptied, cleaned and sanitized before use.
- No sign or poster posted at the hand wash sink in all of the bathrooms to remind food employees to wash their hands. COS
- Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Observed an accumulation of food debris and soil on the floor under the cook line. COS
