McDonald’s on City Line and Haverford Ave in Philly shuts down Monday after failing inspection, 17 violations

Philadelphia’s Office of Food Protection conducted an unannounced retail food inspection on December 9, 2019. As a result of that inspection, and due to the severity of the specific violations the McDonald’s on City and Haverford AVE was cited as being, “Out of Compliance” with Pennsylvania’s food safety regulations. During this inspection a total of 17 violations were found.

As a result the inspector wrote in the report, “This inspection has revealed that the establishment is not in satisfactory compliance. Corrective action is required to eliminate these violations. Compliance status will be assessed upon reinspection. Due to conditions observed during the inspection, the establishment has agreed to discontinue food operations and voluntarily close until it is approved by the Department to resume operations.”

The inspector made the following comments in support of the violations found during the inspection:

  • The food facility operator shall be the Person in Charge or shall designate a Person in Charge and shall ensure that a Person in Charge is present at the food facility during all hours of operation. *PIC arrived at time of inspection.  Corrected On-Site.  New Violation.
  • Black residue, pink slime observed on the interior of the ice machine bin.
  • Beverage nozzles observed with mildew accumulation at time of inspection. *Items were washed and sanitized at time of inspection.  Repeat Violation.
  • Food employees observed donning single use gloves without a prior hand wash.  Repeat Violation.
  • Milk observed held past the use by date inside of the reach-in refrigerator at time of inspection. *Items were discarded at time of inspection.  Repeat Violation.
  • Food (hamburgers, chopped onions, and bacon) observed uncovered inside of reach-in freezer and refrigerator at time of inspection.  Repeat Violation.
  • Clean food equipment observed stored directly on the floor behind front prep area. Cleaning and sanitization needed.  Repeat Violation.
  • Rear outer opening in the food facility does not protect against the entry of insects, rodents, and other animals. Metal door sweep needs repair.  New Violation.
  • Food employees observed not wearing or wearing ineffective hair restraints, such as nets, hats, or beard restraints.  New Violation.
  • Walk-in cooler floors observed damaged at time of inspection.
  • Kick plate observed missing on front under counter refrigeration equipment at time of inspection.
  • Smoothie machine observed leaking water onto counter and floor and not functioning properly at time of inspection.
  • Walk-in cooler door observed in disrepair and not closing properly at time of inspection.  New Violation.
  • Food facility lacks sanitizer test strips to monitor sanitizer concentration. *Test strips were located later on during the inspection.  Corrected On-Site.  New Violation.
  • Food equipment and shelving throughout establishment observed with grease and food debris and needing cleaning at time of inspection.
  • Microwave oven observed needing cleaning at time of inspection.  New Violation.
  • No hot water of at least 100 degrees F observed at hand wash sink in prep area.  Repeat Violation.
  • Broom was observed stored directly on the floor rather than elevated to prevent harborage of rodents and insect.  Repeat Violation.
  • Missing/ damaged floor tiles observed in entryway of walk-in cooler at time of inspection.
  • Low grout and damaged base coving observed in ware wash area at time of inspection.  New Violation.
  • Hood filters have an accumulation of static dust and grease. Vents observed with accumulation of dust inside of walk-in cooler at time of inspection.  Repeat Violation.
  • Light observed inadequate and missing in rear storage area at time of inspection.  Repeat Violation.
