The Chester County Health Department conducted an unannounced retail food inspection on 8/12/2020. As a result of that inspection, and due to the severity of the specific violations the Next Level at Green Meadows Rehabilitation & Nursing Center was cited as being, “Out of Compliance” with Pennsylvania’s food safety regulations. During this inspection a total of 6 violations were found.

This marks the third, “Out of Compliance” regular inspection for the business in almost 2 years:

The inspector made the following comments in support of the violations found during the inspection:

  • One large carton of expired potato salad observed in walk-in refrigerator. Discard this product. Routinely check dates on food containers.
  • Dead and alive roach-like insects were observed in pest monitors and floor drains along the mainline and food prep area. Facility currently receives weekly pest control treatments. Facility and pest control operator must take more aggressive measures to control, reduce and eliminate the roaches. Complete/continue the following: * Certified pest control operator & Green Meadows/Next Level must submit a NEW ACTION PLAN to sufficiently and aggressively address roach activity * Pest control services must continue weekly. Submit copies of pest reports to Department PROMPTLY after service. * Maintain facility surfaces, plumbing and food service equipment in clean condition and good repair * Inspect incoming shipments for vectors * Keep all foods covered in storage * Eliminate standing water and other vector harborage conditions. * Keep exterior openings closed and tightly sealed * Remove any/all roaches (if found) and disinfect the affected area. * Follow pest control operators instructions for cleaning the treated areas and not disturbing the monitors and pesticides.
  • Heavy ice accumulation observed in walk-in freezer. Remove ice build-up and have unit serviced. Latch was repaired at time of inspection on order for door to self-close and seal tightly.
  • Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 0ppm, and not 50-100 ppm as required. Warewash equipment in 3-bay sink until dishwasher can be repaired and operated properly. Inform/remind staff of daily testing of sanitizer using test strips.
  • The Person in Charge does not have adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Review food code and retrain staff. Increase oversight in kitchen.
  • Clean the floors throughout the kitchen, under all equipment, especially at the floor/wall juncture. Clean all floor drains.
  • Replace all stained ceiling tiles in the kitchen area.
